Comparison between analgesic effect of ultrasound guided interscalene block versus shoulder block versus suprascapular nerve block for arthroscopic shoulder surgery: A randomized controlled trial

Background: Shoulder arthroscopy is associated with severe post-operative pain, which causes significant discomfort to the patient, interfering recovery and rehabilitation. The aim of this work was compare efficacy intra post operative analgesia inter-scalene block (ISB), shoulder (SHB) suprascapular nerve (SSNB) for arthroscopic surgeries. Methods: This prospective randomized controlled double blinded research carried out on 90 cases, American Society Anesthesiologists (ASA) physical status I II, admitted under general anesthesia (GA). Cases were equally to: Group I: underwent ISB by 20 ml levobupivacaine 0.5%, group II: SHB 10 0.5% injected below supraspinatus (SS) fascia another at axillary III: SSNB 0.5%. All blocks ultrasound guided, before induction GA.Results: Visual Analogue Scale 2, 4, 6, 12, 18 24 hr, number cases required rescue fentanyl intraoperatively intraoperative consumption postoperative meperidine significantly higher in versus groups (P value < 0.05) insignificantly different between SHB. Time first analgesic requirement earlier (p<0.001) Postoperative nausea vomiting (PONV) (P= 0.031), hemi-diaphragmatic paralysis Horner syndrome (p<0.001).Conclusions: Both have similar hemodynamic stability, analgesia, incidence PONV cases’ satisfaction. showed inferior results these parameters both ISB. However, more complications SSNB.
